Job Title: RN Coordinator Rehab Unit

Salary: $27-$34 per hour

Position Overview:

RN Coordinator will work with the Unit Manager in conjunction with the with assigned unit LPN to assist with treatments, documentation, admissions, care and order transcription.

Essential Duties & Responsibilities:

  • Assist the Unit Manager in coordinating and delivering care to residents
  • Work collaboratively with the assigned unit LPNs to ensure effective communication and coordination of care
  • Assist with the implementation of resident care plans, including administering treatments
  • Responsible for maintaining confidentiality of resident and facility information
  • Review 24-Hour report sheets and follow-up on any issues, admissions and resident needs
  • Responsible for monitoring prescription refills and resolving out of stock medications daily
  • Documentation of resident care activities accurately and timely
  • Responsible for entering weights, vitals and ensure timely completion into PCC.
  • Follow-up on residents who are in the hospital and doctor’s appointments
  • Assist with medication pass as needed
  • Effectively communicate with residents, staff, families and providers
  • Adheres to all fire safety & evacuation procedures.
  • Other duties as assigned.

Basic Duties & Responsibilities:

  • Coordinate daily services among three shifts.
  • Good judgment is required in order to make independent decisions.
  • Complete necessary treatments per physician order.
  • Administer injections and follow on physician orders.
  • Perform glucose testing and administer insulin to diabetic residents.

Education Required:                      

  • Licensed Registered Nurse.
  • Ability to read, write and speak English.
  • Strong communication and supervisory skills

Hazards:                             

Exposure to the following may be present in this position:

  • Blood-borne pathogens and infectious body fluids/waste.
  • Needles.
  • Combative residents.
  • Disinfectants and cleaners.
  • MSDS sheets are available for all chemicals used.


Equipment Used:

  • Glucometers, O2 regulators, blood pressure machines / cuff, stethoscope, catheters, bladder scanner, coaguchek machine.
  • Syringes, needles
  • Photocopier
  • Fax machine
  • Computer

Working Conditions:                     

  • Hours: 9:00am -5:00pm
  • Weekends and holidays may be required as needed
  • Overtime as necessary
  • Working conditions for this position may include the following: inside, odors, adequate lighting, working with/around others, working alone.

Physical Demands:

  • Moderate lifting required at times:
    • ~25 lbs, @ 5% of time.
    • <25 lbs, throughout the day.
  • Constant walking, standing, turning crouching, sitting, talking, hearing, seeing, and concentration are required for this position.

Personal Protective Equipment & Uniform Requirements:

  • Uniform / Scrubs
  • Appropriate Personal Protective Equipment when performing personal care, injections, dealing with blood or body spills, and working with chemicals (cleaning agents).
  • Bethany Village Photo I.D. Badge.
